Overview
First Contact Clinical strive to make a difference to the health and wellbeing of disadvantaged people and communities by allowing a healthy behaviour change.
They specialise in delivering person-centred Behaviour Change services and Skills Training to people and professionals.
First Contact Clinical support several partners and pathways across South Tyneside and have organised capacity into place-based personalised care teams.
They also have a gateway team, which is a team of workers who support locality wide pathways.
The personalised care teams support individuals using a person-centred approach using Patient Activation Measure (PAM), where appropriate, to understand a person's level of activation.
These teams are made up of Social Prescribing Link Workers, Health & Wellbeing Coaches and Care Coordinators.
Support available
- increasing knowledge, skills and confidence, allowing long term sustainable behaviour change
- developing coping skills and strategies for when things aren't going so well
- supporting individuals to identify any unmet non-medical needs including but not limited to social, emotional and financial needs
- engaging individuals with services that can help meet these identified needs
Who can access the service
- people with long-term conditions
- people who are high intensity users of unplanned services, in primary care and secondary care
- people with non-medical needs, i.e., social and emotional needs
- people who are supported by the Integrated Care Teams and Social Care Teams
- veterans
- people at risk of offending or re-offending
