What happens when you contact the team
The team will answer queries or connect the person to support available in their community in order to improve their independence and achieve the things that matter to them.
If after the first contact with the team , a more in-depth, face-to-face discussion is needed, we can offer appointments in a community base such as The Word and Hebburn Central.
If you are a carer, you may be offered an appointment at South Tyneside Adult Carers Service to connect you with support available specifically for carers, as well as wider networks to help you to continue with your life in the way you wish to.
If you are unable to attend one of the venues currently available, we can arrange for a home visit to be made where necessary.
